Uninsured? A $200 payment will be due when you check-in for your new patient appointment.

Within 24 business hours of receiving your request, we will call you to schedule your appointment.

What We Need From You When We Call
Please be prepared to provide us the following information:

Names of your primary care and/or referring physicians.

Your active health insurance plan(s) details: ID/subscriber number; group number; guarantor's name.

Your demographic information: Name, date of birth, address, etc. (refer to and complete Omaha patient forms, Lincoln patient forms)

Scheduling your new visit may take 10-20 minutes by phone.
